Q: is this truly a tension rod or does it need to be screwed into the wall?

It is a tention rod no screwing into wall.

Q: The covers pop on the mounting bracket but there aren't any set screws. Covers clatter back and forth. Very annoying. Did anyone get set screws?

Q: Will this hold on ceramic tile walls?

Yes, so far ( 2 weeks ) I have not had any trouble with it slipping down the walls.

Q: Would this work to install on sheetrock or does it need to be tile or fiberglass tub surface?

I don't think it has to be tile. as a matter of fact, I could not get to stay in place on my tile, but it's not a totally smooth tile. It was a "suction like" that needs a smooth surface to stick to when you tighten.
