Question 1

Has Medline fixed the issue of the 3-spoke vs. the 8-spoke wheels. If not, I am going to buy the Invacare unit that has 6-spoke wheels.

When I contacted Home Health SuperStore because the walker I received was not that displayed on either the amazon or Medline websites, Helen, the store's superb customer service representative, handled the matter effectively and efficiently and with uncommon concern for my satisfaction.  She immediately researched the issue, called back when she said she would, and offered options.  She contacted Medline and learned that, despite amazon reviewers' complaints about receiving walkers with three-spoked wheels dating from 2012, Medline had simply not bothered to update product photos on either site.  Because of Helen's outstanding service, I'll buy products from Home Health SuperStore, but they won't be Medline products.

do the brakes set so the walker doesn't move when sitting?

Yes.  The brakes firmly clamp and the walker will not move when sitting.

Is the backrest a rigid bar?

A foam covered rigid bar. Side note: this thing works great for my 390lb 83 year old father.

does it arrive assembled?

If you have any trouble assembling this sort of thing, get someone else to do it.  The one I got , which is intended for my disabled brother's use, came completely DISassembled.  The instructions are about as clear as clay.  They didn't even say to install the rear wheels!!!  Which, by the way, had instructions on a tiny tag stuck on the wheel post (if you need reading glasses, add a magnifying glass to your toolkit for this).  The rest of the instructions are extremely vague, and don't even say to remove the plastic grommets from the wheel shaft openings (I thought at first they were bushings; they're just packing material).  Make sure you put the thumb rests toward the inside of the stroller.  There are clips on the legs to slip the brake cables into.  The "circular screws" (supposed to have 4) and the knob-type screws (2) were all the same (6 knob-types) so don't go looking for different screws, just count up to 6.  Good thing I used to be a bicycle mechanic, because I had to figure this out all on my own.  The "assembly instructions" are worse than worthless.
