How often should the traps be replaced?

When they become empty. You can usually tell this when there are no ants trailing in, but there are still ants near by.

Once opened, how long do they last/are they effective for?

i received them in the afternoon and spread them around my kitchen as soon as I got them. By the next morning (when they were usually the most active) they were almost all gone. Maybe 2 or 3 instead of an army. By the following day they were gone. I decided to leave them in place for awhile since they are out of sight. It did the job in a day, after that, what does it matter?

I literally got an empty container. Did anyone else have this?

I have ordered twice. No empty container, but the grandules are small on the bottom. You just punch the 4 holes in the sides and put and watch the ants die.

Does this work for flying ants ?

Unknown. We do not have flying ants. This ant killer certainly took care of the low crawlers! (But, then.maybe we had airborne ants and the product annihilated them!)
