Is there a metal stay in this brace to keep your wrist from bending while wearing it?

It isn’t metal but there is something so that the wrist will not bend and its strong. Your wrist will be very steady.

I should immobilize my index, middle finger, thumb, and wrist. Is this supportive and firm enough?

Yes, it does not give. However, my fingers sometimes slip out while sleeping. You can adjust the straps which helps. One strap is completely detached from the brace. I can usually position it to keep my thumb from slipping out. The key for me is finding a happy medium between being too tight and not tight enough.

How do I know what size to order for a 86year old woman?

They already have size of hand splint S, or M or L on their page. You need to look what her old hand splint is and easily find a right size. That's what I did for my husband.
