Was able to pressurize system enough to diagnose.
The Mityvac MV4508 adapter was able to pressurize the system but not beyond 18 psi. This was enough to test the system, even though the cooling system in this application was rated for 20 psi. Beyond 18 psi the adapter would slip and pop out. Non of my specialized adapters would fit in this particular instance..

Bad design or faulty Manufacturing
Every time I try to use it it pops out, bad design or faulty Manufacturing, I disassembled it, and modified it to work by putting the rubber on the opposite direction, no more popping out.

Okay
The older style works better, so I found and bought that one too. This one works okay, but does slip easier. And it's harder to tighten. We're trying to find coolant leaks... how can we expect to get the surfaces 100% dry before using it? I'll keep it for the rare case my other fittings don't fit.

Good but read instructions
Make sure that the fill hole on the radiator is dry or this thing will smack you in the face or hit the hood of the car (I’ve seen it happen when I let other mechanics borrow this) it doesn’t fit all radiators like jaguar, vw, and some GMs with the pressure cap on the reservoir
