Yes this model is for a on/off switch only you need taymac mm410 that is 2and3/4 deep or mm510 which is 3and1/2 in deep depending how heavy your cord is . ihope this helps

It is a plastic on plastic hinge.

I don't know. I would check with the city elect. codes. This outlet is designed to go outside. It will keep the rain and snow out.

Can be use with the 480V 20 A plug?

This cover goes over a standard single gang elec. box, and raises to slightly over 90 degrees. So yes I'm pretty sure your plug will fit - but I seriously doubt the cover will close completely with anything plugged in. I can't normally close mine with a standard extension cord plugged in, but short of a serious downpour the lid provides enough "cover" that my GFCI never trips.
