Q: will this fit the ender 5 plus

Works great, they will fit all. Simple but reduces sound a lot. I have them on all my Creality

Q: These come in set of 4. How do you mount on Z steppers?

On my Ender 3 pro, I was only able to use it on the extruder motor and the bed motor, to use them on the Z and X you have to make modifications or reverse the motor mount which requires a bootloader so you can tell the printer to reverse the motor. If oy have a newer Enbder 3 pro you don't need these honestly. They are quieter and smoother now.

Q: Does this come with thermal pads as shown in the picture? Will this work with pressed-in pulley with no extra mounts necessary?

Yes, I order and received 4 dampers, 4 thermal heat sinks and 4 sets of screws. Pretty easy install too.
