Do these come in a plastic box?

Yes. It has a black cardboard outer box and a white plastic inner box that contains the magnet keychains.

2pcs per box? Can the two magnets stick together perfectly?

Yes, ordering a second pair. Very strong!

whats the difference between 12mm or 15mm ? Thanks

3mm, or about 1/8 inch. These are small magnets to begin with. The pictures do not show the actual size well. When looking at them side by side there is an obvious difference in size, but it is not very significant. It all depends on what you intend to use them for or what size opening you need to put them through.
