It´s good for a men shoe size # 15 ?

No. The medium size will not fit a mens size 15. I have a size 8 womens and used this when I hurt my foot. It will not fit my boyfriends foot and he is a mens size 9.

Is this covered by insurance?

Not through amazon you would have to go through a specialty store that have them stocked but to be honest I doubt Insurance pay for it I had to pay out of pocket

i wear size 8. do i need small for this?

Depends if you are ordering for a Man or Woman.. Small fits a women's 6-8, Medium fits Men's 7.5-10.5

Men's shoe size 8-8.5, is small the correct size to buy?

NO, MEDIUM Mens 7-10.5 / Womens 8.5-12
