Where is this powder sourced? I know it says packaged in Utah, but where is it sourced from originally?

-We've thrown away the bag, as it was purchased a while back. As I recall country of origin wasn't stated. However, if you aren't taking one brand on a consistent basis that should help reduce any associated risk factors, if applicable.

Is the measuring scoop included?

no, but i saw in youtube quarter teaspoon is a good dose?
