It's a skinny bottle, should fit most cup holders.

Are there replacement caps for the bottle?

I don't think there is I haven't seen any.

I believe it was mfg in Finland, but not sure as I am not associated with Jarlson. That said, we are very pleased with the quality of the water bottle.
