How do you disinfect it and prevent mildew?

I add hot water, dish liquid soap, and a little bit of bleach, shake it up and let it sit. Then rinse it out.

Is the straw above the latch so your lip doesn't touch it?

So that top plastic piece is a latch and the river part is the straw. You close the latch and it closes down the state and locks so it doesn’t leak. Then there’s a button on the side to pop it open. I hope this answer was helpful.

Does it make noise (squeaky kind of noise) when you drink from the straw?

It make an airflow sound when drinking from the straw. To stop the noise you just have to loosen the cap a little and the noise goes away.

Does it fit in a car cup holder?

Depends. Not all cup holders are created equal. It's about 3" across at the bottom. Fits the car holder but not the pickup.
