Does this only mirror screen to another monitor?

Yes, it allows one source to be distributed to other monitors, mirroring each other. I use this to be able to split one HDMI outlet on my cable box to a 2nd TV I have at my computer workstation in the same room as my 40" TV.

if I extend an image onto this adapter will it output 2 mirror images of that extension

This will mirror the image from an input HDMI source to two HDMI displays.

would this item work run my both projectors. I want to extend the picture signal from my Android box or would you know of a product that will

This will not extend the picture. It will show same picture on both projectors.
