Can this be fitted to a standard back to wall toilet with a concealed cistern? I've hot & cold outlets available in wall for a douche but want this...

I had to return this as all the fittings are American even though it says it can fit a UK toilet. A plumber even tried to source parts but they don’t use the same measurement even though they’re similar

Can you clever people please tell me how is the hot water t-adapter connected to the uk pipe? ours is not 3/8" and can't find a 8mm hex nut!

We had to send ours back because of that same problem

Can you install it on a closed toilet?

Not sure what you mean by "installing on a closed toilet"..if you're asking "Can the seat cover still close completely after this is installed?" then the answer is yes; if you're asking "Can it be installed with the seat cover kept in the closed position?" then the answer is no, because you will be unscrewing and completely removing the seat + cover while you install this, then screw them back on..

Can someone please explain what additional components i’ll require to make this compatible for use in uk? thanks

Nothing specific for the UK (that's where mine was installed), just compatible wrenches tape etc for the specific toilet/waterpipe setup in your HOUSE (not shouting, just no italic font to emphasize the fact different houses will have different fittings depending on their age!) The box contains the basics, but if you have non-standard pipes you might need an additional adaptor or two from B & Q for less than a pound each..
