How many 45s fit on this? Looks like only 3 of the 45 bumpers would fit on either side? What about iron 45s?

Depends which 45lbs bumper plates you're using. I use urethane plates and can fit 4 each side. Wider bumpers I can probably only see 3 fitting. Iron plates I would say 4-5 depending how much collar space you need. Calibrated comp plates maybe even 6.

Is the distance between the grips really 20"?

24" from the inside of the grips.

Anyone about 5'10"? It looks like it wouldn't clear my knees and rear if I do a squat. Did anyone have similar issues?

I am the same height as you. I didn't have this problem. It works better if you flip the bar for squats to where the "raised" handles are on the bottom. I've had no issues either way though.
