one side is 100mm proud of the tyre surface, the other is 40mm below the tyre surface

Q: what size are the nuts on the hub bolts

They would appear to be M8 bolts with M13 nuts. However, the M13 spanner is very tight, thus they may be imperial measurement of nine-sixteenths

Q: Are these actually solid rubber all the way through, or just just hollow plastic ?

As far as I know, they are solid plastic - certainly by the weight they are. I had builders here who used my trolley from time to time and there was never any thought of the tyres bursting. However, they did rather overload the trolley and actually distorted the wheel centres, but to be clear, that was no fault of the wheels.

Q: Do these wheels come with bearings already built in

No bearings as such, just sleeves. The wheels slide on the axle.
