Q: The small screw in the middle of the bar came out. The bar cannot expand or retract without slipping.

That is one of my issues too! I removed bar from brackets and rotated ends in opposite directions shortening the bar until I saw a corresponding hole on the inner bar. I still had the original screw but think any properly sized screw (set screw maybe better option) would work. Then twist ends to lengthen bar. I searched instruction sheet and there is no mention of this screw.

Q: Why do the directions say this is not to be used for chin ups and only for situps and crunches?

Bar comes with two mounting systems one way for chin-ups one for sit-ups

Q: Can weights be attached as on a barbell?

I wouldn't do that. They could fall off and smash your foot.

Q: Can I close the door after installing the brackets?

If you instal it on the outside of the door jam it does not interfere with the closing of the door.
