When I bought mine it was included. But the quality was so bad I wish it wasn't... It smears forever. YMMV.

No. Just 0-9. I found if you put a wheel in-between digits it will print a blank, but IDK if that's on purpose or if I'm abusing it.

Q: Does this item arrive ready to use, or do I also need to buy an ink/ink pad etc.?

It had a bottle of ink and two pads included with the stamper.
