Q: Is the pinhole small enough to allow one single drop to fall?

The pin hole is not for drops to fall through. It's for the patient to look at while administering drops from the bottle. It works well by keeping the patient from flinching as the drop falls from the bottle into the eye.

Q: I use a small bottle every night, would it work with snmall bottle ?

Yes. It worked fine with my Mom's 3-4 small bottles of eye drops. It's got a catch on it that holds onto the neck of the bottle.
