Aren't both ends supposed to light up.

Only the end you plug the tool into. The end that you plug into the power is clear with no light.

Would this hold for 6.5amp/115v fridge?

Yes, I used it to power a surge protector that fed 2 fans, 2 lamps ,weather station & refrigerator during the recent Hurricane

how thick is the 12/3 Gauge Heavy Duty outside diameter

The best answer is 7/16 of a inch, hopes this helps. Also the outside jacket of insulation is very good, very happy with the cord.

Why is this 12/3 extension cord only rated for 15A?

It doesn’t have the 20 amp recepticle ends and there’s more than a 3% voltage drop over the 100 feet.
