Can it handle the weight of a Juki tl-2010q?

The cabinet is not sturdy and will wobble when shaken. Don’t know the weight factor of your machine, but if it’s power, expect the cabinet to shake even with the wheels locked. Mine is on carpet and haven’t sewn yet, but I can shake the cabinet with my hand.

Does the lower shelf hit your shins?

No it doesn't. It's perfect to hold your machine.

replacement casters, is there a way to replace the casters, I haven't even used this table and the casters are falling apart?

Either glue them in or put a piece of tissue and then push the casters in
