Yes, there is a video showing how it can be worn on the left side. First, you insert the umbrella into the holder, then place the holder flipped (with the attached umbrella) on the left shoulder with the umbrella being on the backside of the shoulder instead of the front.

How large a chest/shoulder will this fit?

It fits a small to medium shoulder/chest size without straps. And a large to extra large size with the included straps. The holder comes in a compact form and flexes open (by stretching it) to accommodate larger chest sizes.
