Will it actually hold 4 four bikes for sure?

We have it on our minivan and it was tight with 3. I would definitely be concerned with 4. As it was, my chain got pulled and one of the bikes got scratched from putting on 3. We have 2 mountain bikes and a cruiser. You will need to purchase additional items if you have a cruiser and a woman's bike.

Would this fit on a 2012 Subaru Outback?

Yes, this would fit, but you would need to have the appropriate trailer hitch installed. I have this rack on my 2004 Forrester, and I had the hitch installed at U-Haul for about 200 including parts and labor.

As of 2018, does this still only hold 3 bikes?

Tough question ... there are straps for 4 bikes on the Yakima that I bought, but I, personally, would have a hard time fitting 4 bikes on it -- I've only done 2, and think 3 will work, but be tight.
