They did come with brief instructions but a user can probably figure many exercise routines on their own. Just know your limits!

What's the diameter, meaning how thick is each bar?

The diameter is slightly bigger from red, to green, to blue. This is how the resistance (force needed to twist or bend) is increased.

Works for tennis and golfers elbow!

I had tennis elbow and used the red one as directed and within 2 weeks my elbow felt good enough to golf. After about 4 weeks, I moved up to the green one to strengthen the elbow. I don't think I will ever make it to the blue one, time will tell.
