Q: How well does this work on a lawn? Does the resistance wheel slip? Does it need extra weight and how much?

This works on all surfaces, additional weight by the front wheel will assist with traction and keeping resistance constant. A harder surface like a lawn, would have no issue. You may need to add additional weight on a looser surface say a lawn with dew, dirt or gravel.

Q: does pulling the sled backwards offer resistance? same as pushing it?

Yes and in addition to adding plates there are 3 gears you can manually adjust. Pushing faster also seems to add more resistance. I train firefighters with it. I even bought a heavy duty rope with a clip to use it as an upper body pulling exercise.
