Pair it with a TRS cable
I paired this with a TRS cable I already owned, and now I'm able to control MIDI switching on my Strymon Iridium using my Boss MS-3. (Strymon makes a cable to do this, but it's expensive.) Instead of being limited to 2 channels on the Iridium, I'm now able to program as many as 300! (Obviously way more than anyone needs, but having 4-6 different sounds out of the Iridium is great.)Some people have reported that they can't get this to work, but it's probably because they are using a guitar patch cable instead of a proper TRS cable. TRS cable has 2 rings on the plug, and you need to use it if you want to communicate MIDI messages. As far as this MIDI converter is concerned, it works great!

Works great for guitar pedals that take MIDI over TRS
Works great for my Meris Enzo. Meris pedals take MIDI over TRS. My Boss ES-5 sends MIDI over 5 pin DIN. I didn't want to spend more money on Meris's MIDI I/O just to control the Enzo so I bought this and it works like a charm. I plug the 5 pin end into the MIDI controller and connect the TRS jack to the Enzo with a TRS cable and I can MIDI control my Enzo without having to buy additional hardware.

Can't use for expression pedals
I bet this works fine if the device you use is powered. This however will not work for passive expression pedals.

Nope
For the function I wanted out of it, it didn't work. I tried connecting an amp through a 1/4" 'guitar' cord to my electronic drum set and got all sorts of feedback. No drum sound made it through to the amp at all. Glad it was cheap! Might try it somewhere else in my music room...
