Q: If you have 4 or less bottles in this 6 bottle bag, will they all shift around or does insert stay in place?

I purchased the 4 bottle bag and the bottles don't shift when I carry less than 4.

Q: Has anyone with kids used this bag for water bottles?

Nope, but I think it would work pretty well, because I've carried six glass wine bottles in it with no issue and water bottles would be lighter. The only consideration is that the interior separators aren't attached to the bag, so if you only had a few bottles, they might tip over.

Q: Is the shoulder strap long enough to carry bag cross-body?

Sorry I cannot answer your question as I do not use the strap and I cannot locate it to measure.

Q: can this item be embroidered? (not by the seller but somewhere else?

Not sure because the material al around is thick as the inside has the insulation material. It is the silver material inside plus some foaming padding.
