How far down does the neck go?

Not at all. It just covers your chin. However, it is not very noticable. Very comfortable to wear.

Would this mask fit a 5 year old?

Unless the 5yr old has a huge head, I think not

No the mouth does not open. Great look, but needed to cut more breathing holes and en then it was not easy to breath comfortably

Does the hair cover the whole back of your head?

No. The hair is stringy on the sides. I bought the mask for a friend who does a Halloween display. Really nice mask for a witches effect.
