You would need ice packs to keep it cool. Its not cold on its own. It is just a bag.

No. I pop the bag in the fridge before using and that seem to work.

How long will this keep insulin cool for? Need a cool bag for an overnight stay will have no frodge aceess so need vial to stay cool overnight
